Wrong units, skipped steps, misreading "how many more" as "how many" — these look like attention problems. They're usually incomplete understanding disguised as carelessness. The fix for each is completely different.

They can calculate, but can't interpret. They read the question three times, don't know where to start, and either guess or shut down. Most students have never been taught when to draw a model versus when to just calculate.
